Purpose and Scope of the Information Text

As Murat Aktaş Health Center Turkey, we take measures to protect your personal data under Personal Data Protection Law No. 6698 (“KVKK” or “Law”). We process your personal data in accordance with the KVKK and related legislation, acting as the “data controller,” for the reasons and methods described below.

The Information Text on the Processing of Personal Data has been prepared in accordance with Article 10 of the Personal Data Protection Law, titled “Data Controller’s Obligation to Inform,” to provide you with the most transparent information regarding the identity of the data controller, the method and legal basis for collecting your personal data, the purposes for which this data will be processed, to whom and for what purposes it may be transferred, the duration of data processing, and your rights as listed in Article 11 of the Personal Data Protection Law.

The explanations provided in the Information Text regarding “Your Personal Data” also cover your “Special Categories of Personal Data.”

Method and Legal Basis for Collecting Your Personal Data

Your personal data may be collected through automatic or non-automatic methods, including through the company’s affiliated units, website, social media channels, call center, mobile applications, software used to conduct company activities, camera recording, and verbal, written, or electronic means.

Your personal data may be generated, updated, processed, and stored both digitally and physically for as long as your relationship with the Company continues.

Your personal data may be collected for the purposes listed under “Processed Personal Data and Purposes of Processing” and provided that the explicit consent requirement is met under Article 5, paragraph 1, of Law No. 6698, or as specified in Article 5, paragraph 2;

a) It is expressly provided for by law, b) It is necessary for the protection of the life or physical integrity of the person who is unable to give consent due to a physical impossibility, or whose consent is not legally valid, or of another person. c) It is necessary to process personal data belonging to the parties to a contract, provided that it is directly related to the establishment or performance of a contract. ç) It is necessary for the data controller to fulfill its legal obligation. e) Data processing is necessary for the establishment, exercise, or protection of a right. f) Data processing is necessary for the legitimate interests of the data controller, provided that it does not prejudice the fundamental rights and freedoms of the data subject.

Personal data is processed within the scope of processing conditions.

Your special personal data, processed for purposes other than health and sexual life, such as “association membership or union membership,” will be processed in accordance with the “Processed Personal Data and the Purposes of Processing,” for the purpose of benefiting from special discounts offered by associations and unions with which our company has agreements. This data will be processed in accordanceance with the requirements of explicit consent, paragraph 2, Article 6, of Law No. 6698, or in the cases stipulated by law pursuant to paragraph 3, Article 6.

Your special personal data, processed for purposes other than health and sexual life, such as “religious information,” will be processed in accordance with the “Processed Personal Data and the Purposes of Processing,” for the purpose of determining your preferences regarding blood transfusion and for the purpose of carrying out the process in accordance with your sensitivities during the medical process. This data will be processed in accordance with the requirements of explicit consent, paragraph 2, Article 6, of Law No. 6698, or in the cases stipulated by law pursuant to paragraph 3, Article 6.

Your special personal data regarding your health information will be collected for the purposes set forth under “Your Processed Personal Data and the Purposes of Processing,” in accordance with the requirements of explicit consent under Article 6, paragraph 2, of Law No. 6698, or solely by authorized persons under a confidentiality obligation pursuant to Article 6, paragraph 3, for the purposes of protecting public health, providing preventive medicine, medical diagnosis, treatment, and care services, and planning and managing healthcare services and financing.

In this context, the data listed in the table are collected as personal data:

• Identity: This is the data group containing information related to individual identity. (e.g., Name, Surname, Date/Place of Birth, Turkish ID Number, Gender, Social Security Registration Number, Nationality, Marital Status)
• Contact: This is a group of data that can be used to contact a person. (e.g., Phone Number, Residential Address, Email Address)
